Role Overview

A dementia professional is required to join our team in delivering a Dementia Support Program. This role involves working collaboratively with colleagues to improve lives and implement best practices.

Key Responsibilities
  • Support individuals living with dementia, their families and caregivers by understanding how dementia impacts each person.
  • Equip carers and clinicians with the skills and knowledge to prevent and respond to behaviour changes through modelling, coaching and resource development.
  • Develop reports contextualised to individual situations that effect change on the ground and improve quality of life for people living with dementia.
  • Undertake assessments to understand the holistic experience of the person and their care network.
Required Skills and Qualifications
  • Experience supporting people living with dementia, with a genuine commitment to supporting individuals and their families.
  • Relevant qualification in Allied Health, Nursing, Aged Care or Mental Health.
  • Excellent communication, negotiation and relationship building skills.
  • Ability to prioritise multiple tasks and meet deadlines.
  • Empathy and patience.
