
Community Support Specialist

About Us:
We are seeking an experienced Care Partner to join our Community Aged Care Services team.
The role will be based in Queensland, with travel required to the Southside of Brisbane.
The Care Partner is responsible for delivering care management that enables older people to live safely and independently at home.
They will collaborate with participants, their supporters, and interdisciplinary teams to coordinate tailored, responsive, and inclusive support.
This role involves:
- Collaborating with participants to develop individualised care plans within required funding timeframes
- Incorporating participants' goals and environmental requirements into care plans and delivery
- Establishing and updating care plans and funding requirements, ensuring participants understand their services and financial responsibilities
- Reviewing care notes and service delivery data
About You:
To succeed in this role, you will possess:
- Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent
- Demonstrated experience in care coordination, case management, or similar roles in aged care, health, or disability services
- Strong communication, empathy, and cultural competence
- Skilled in using care management or CRM systems
