What you'll be doing
- Provision of care coordination for older people who present with complex mental health issues across 2 sites. This may include physical co-morbidities, psycho-social challenges and issues of addiction of substance use.
- Provide education and guidance to internal and external partners including Residential Aged Care Facility staff.
Candidates Will Need To Meet The Following Criteria
- Registered Nurse with a minimum of 5 years full-time post-graduate experience in a mental health specialty. Must have an approved post-graduate qualification within mental health, drug and alcohol, or other qualifications/experience deemed appropriate.
- Demonstrated experience working with consumers and carers presenting with complex mental health needs and/or behaviours of concern; ability to assess, plan, implement and evaluate interventions and care plans.
- Extensive experience in conducting comprehensive mental health assessments and developing, implementing and evaluating consumer-focused care plans based on evidence-based practice.
