Job Title: Senior Care Coordinator
About the Role:
We are seeking an experienced Care Manager to join our team in Gold Coast. The successful candidate will be responsible for coordinating care services for clients with aged and disability needs.
Key Responsibilities:
- To work collaboratively with clients, families and healthcare professionals to develop and implement personalized care plans.
- To conduct assessments, reviews and updates of care plans as required.
- To manage client budgets and ensure efficient use of resources.
- To provide advocacy and support services to clients and their families.
- To work closely with multidisciplinary teams to ensure seamless delivery of care services.
Requirements:
- A minimum of 3 years' experience in care management or a related field.
- Tertiary qualifications in community health or a related discipline.
- Proven experience in case management, budgeting and client advocacy.
- Strong communication and interpersonal skills.
- A current driver's license and willingness to travel.
