Nurse Care Coordinator for Complex Mental Health

This position offers an exciting opportunity to make a difference in the lives of older people with complex mental health needs. We are seeking a skilled and compassionate Nurse Care Coordinator to join our dynamic multi-disciplinary community team.
About the RoleThe successful candidate will provide care coordination across two sites, managing physical co-morbidities, psycho-social challenges, and substance use issues. This involves educating and guiding internal and external partners, including Residential Aged Care Facility staff.
A key aspect of this role is developing evidence-based, consumer-focused care plans, incorporating mental health assessments and interventions. The ideal candidate will have extensive experience working with consumers and carers with complex mental health needs.
Key Responsibilities- Conduct thorough mental health assessments and develop comprehensive care plans.
- Provide education and support to internal and external partners, ensuring continuity of care.
- Collaborate with multidisciplinary teams to ensure effective service delivery.
- Registered Nurse with at least 5 years of postgraduate experience in mental health.
- Approved postgraduate qualification in mental health or equivalent experience required.
- Extensive experience working with consumers and carers with complex mental health needs.
