Clinical Care Specialist

We are seeking a highly skilled and experienced Clinical Nurse Consultant to join our complex care team. As a key member of our multi-disciplinary community, you will provide expert care coordination for older adults with complex mental health issues across two sites.
You will be responsible for developing and implementing evidence-based, consumer-focused care plans that address physical co-morbidities, psycho-social challenges, and substance use issues.
- To be successful in this role, you must have at least 5 years of full-time postgraduate experience in mental health as a Registered Nurse.
- A strong understanding of complex mental health needs is essential, including experience working with consumers and carers.
- You will conduct comprehensive mental health assessments and develop tailored care plans.
- Your expertise will guide internal and external partners in delivering high-quality care.
- You will lead the management of physical co-morbidities, psycho-social challenges, and substance use issues.
