Work Location Ryde Community Mental Health Centre - 39-41 Fourth Avenue EASTWOOD & RNS Community Centre, 2 C Herbert Street St LEONARDS Ryde Hospital is undergoing a $479 million redevelopment to transform healthcare delivery, including new facilities and enhanced services such as emergency, ICU, inpatient units, and outpatient care.
Construction began in early 2023, offering exciting opportunities to be part of shaping future health services for the Ryde community.
Role Responsibilities Provide care coordination for older people with complex mental health issues across two sites.
This includes managing physical co-morbidities, psycho-social challenges, and substance use issues.
The role involves educating and guiding internal and external partners, including Residential Aged Care Facility staff.
Additional Information People of Aboriginal and/or Torres Strait Islander backgrounds are encouraged to apply.
Candidate Requirements Registered Nurse with at least 5 years of full-time postgraduate experience in mental health.
An approved postgraduate qualification in mental health, drug and alcohol, or equivalent experience is required.
Experience working with consumers and carers with complex mental health needs, including assessment, planning, and evaluation of interventions.
Extensive experience in conducting mental health assessments and developing evidence-based, consumer-focused care plans.
