About the Role:

The successful candidate will be responsible for provision of care coordination, education and guidance to internal and external partners, including Residential Aged Care Facility staff. They must have a minimum of 5 years full-time post-graduate experience in a mental health specialty, with an approved post-graduate qualification within mental health, drug and alcohol, or other qualifications/experience deemed appropriate.

  • Conduct comprehensive mental health assessments and develop, implement and evaluate consumer-focused care plans based on evidence-based practice.">
  • Demonstrate expertise in working with consumers and carers presenting with complex mental health needs and/or behaviours of concern.">
  • Collaborate with multidisciplinary teams to provide high-quality patient care and services.">

Requirements:

  • Registered Nurse with a minimum of 5 years full-time post-graduate experience in a mental health specialty.">
