Our service provides holistic multidisciplinary care for people of all ages with life-limiting illnesses, including psychological and spiritual support, symptom management.
We operate in the Western region of Melbourne, caring for over 500 patients in their own homes.
Our team is a recognised rotation with the college, offering expert guidance on symptom management, psychological, and end of life care support to patients, families, and healthcare professionals.
Responsibilities:- Provide specialist palliative care consultation to other units in a hospital setting.
- Offer expert guidance on symptom management, psychological, and end of life care support.
- Work as part of a multi-disciplinary team, providing compassionate and flexible care.
- A valid medical registration.
- Excellent communication skills.
- Ability to adapt and problem solve.
- A compassionate, flexible, and positive approach to patient care.
