Clinical Care Specialist Wanted

About the Role

The successful candidate will be responsible for delivering clinical care across various programs, including Home Care Package (HCP), CHSP, and other initiatives. Your key responsibilities will include:

  • SUPPORTING CLIENTS WITH CHRONIC DISEASE MANAGEMENT AND MEDICATION ADMINISTRATION
  • COLLABORATING WITH ALLIED HEALTH PROFESSIONALS TO PROVIDE HOLISTIC AND GOAL-DIRECTED CARE
  • PROMOTING HEALTH EDUCATION AND SELF-MANAGEMENT THROUGH TELEHEALTH AND FACE-TO-FACE VISITS
