Clinical Case Manager

A clinical case manager position is available to support residents and SRS staff in managing behaviors of concern and complex co-morbidities.
At Peninsula Health, we are seeking an experienced professional to fill this role. The ideal candidate will possess strong skills in care coordination, public health programs, and client-centred practice principles.
- Conduct comprehensive assessments to identify care goals, address unmet health and social needs, and manage complex care situations.
- Develop expertise to support residents and staff in managing challenging behaviors and co-morbidities.
- Collaborate with healthcare professionals to develop client-centred Care Plans that coordinate services and engage programs building on residents' strengths.
Key requirements for the position include:
- A valid driver's license and excellent time management skills.
- Highly developed communication and documentation skills.
- Experience in case management, care coordination, or public health programs.
- A commitment to trauma-informed practice principles and the Social Model of Health.
