Occupational Therapist Case Manager Role
Job Description:At a leading teaching, research, and tertiary health service, we are seeking a highly skilled Occupational Therapist to join our Aged Psychiatry Assessment and Treatment Team as a Case Manager.
Key Responsibilities:
- Providing psychiatric assessment and treatment to clients in their homes, community settings, and residential facilities
- Responsible for intake and triage, crisis support, assessment, and case management
Requirements:
- Relevant degree and/or postgraduate qualifications
- Relevant Australian Health Practitioner Regulation Agency (AHPRA) registration
- 2 years of post-qualification clinical experience in Mental Health and/or aged care clinical services, including case management and care coordination experience
- Passion for achieving best clinical practice
