To be successful in this role, you will need:

  • A social work qualification and current registration with AHPRA or relevant professional organisation.
  • Experience working with people who are at-risk and vulnerable and have complex needs.
  • Specialist experience and knowledge regarding chronic disease management.
  • Effective communication and interpersonal skills.
  • Ability to manage multiple competing tasks and effectively prioritise.
