Care Coordinator

Our team of compassionate professionals is committed to delivering high-quality care that meets the unique needs of each customer.
Duties and responsibilities include:
- Developing and managing customer service schedules to ensure seamless delivery of care and support services
- Preparing and transmitting schedules for customer services to Community Support Professionals
- Arranging replacement staff to cover planned or unplanned absences and shift changes
- Providing coaching, supervision, support, and direction to Community Support Professionals to ensure quality service delivery
- Monitoring the implementation of individualized plans for customers
- Verifying allocation of Community Support Professionals against customer service schedules to ensure timely processing of timesheet data
- Maintaining and updating data in company information systems
- Meeting specified standards for the delivery of quality in-home care and support services
About you:
- Caring, compassionate, and friendly
- Demonstrated high-level verbal and written communication skills, including familiarity with Microsoft Suite
- Proven relationship management with internal and external stakeholders, as well as good interpersonal, negotiation, and problem-solving skills
