Job Title: Registered Nurse Coordinator
About the Role:
This is a permanent part-time position starting with 3 days per week with the possibility to increase to 5 days. As a Registered Nurse Coordinator, you will be responsible for providing individualized and personalized support to customers in their homes, in accordance with their support plans and/or medical practitioner's directions.
Key Responsibilities:
- Work with or under the guidance of medical practitioners and Allied Health professionals to deliver high-quality care to customers.
- Administer medications and provide complex clinical care, including palliative care, bowel care, catheter care, stoma care, and oxygen therapy.
- Monitor and communicate on customers' wellbeing and report progress to relevant stakeholders.
- Ensure all services are provided in a caring and respectful manner, in accordance with Just Better Care's policy and procedures, legislative requirements, and program-specific guidelines.
- Provide training, coaching, and guidance to Community Support Professionals, Enrolled Nurses, and Support Staff/Case Managers as required.
- Conduct skills assessments of Community Support Professionals performing complex care services as required.
- Demonstrate a commitment to best practice and evidence of continued professional development.
- Participate in staff meetings and attend training opportunities when required.
Requirements:
- Current CPR/First Aid Certificate
- Current and satisfactory Federal Police Check, NDIS Worker's Screening Card, and Working with Children Check Blue Card
- Current Australian Driver's Licence
- Proof of working rights in Australia (e.g., Passport, Birth certificate)
- Reliable, registered, and insured vehicle (comprehensive insurance preferred)
- Smartphone with internet access
- Registered AHPRA
- COVID-19 Vaccination Certificate
