Care Manager Lead

At Ozcare, we are seeking a highly skilled and experienced Care Manager Lead to join our team. As a key member of our leadership team, you will be responsible for providing strategic leadership and guidance to our Care Manager team in the planning, management, and delivery of high-quality client care services.
Key Responsibilities:
- Develop and implement care plans tailored to individual client needs
- Liaise with clients, families, and healthcare professionals to ensure comprehensive care
- Manage and coordinate care services to ensure the highest standards of care
- Ensure compliance with all relevant legislation and organizational policies
- Monitor and evaluate care services to continuously improve quality and efficiency
- Work closely with the Branch Manager and Care Managers to ensure service growth
- Provide supervision and support to the Care Manager team
Requirements:
- Demonstrate experience in case management and client care coordination in a Community Aged Care setting
- Demonstrate strong verbal, written, and interpersonal communication skills to communicate and work effectively with a broad range of people
- Show the ability to develop staff, foster teamwork, and effectively work as a team member while providing leadership
- Provide proof of immunity or willingness to become vaccinated against Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox), and Pertussis (Whooping Cough)
- Hold a current Police Screening Certificate
- Provide current First Aid and CPR Certificate
- Provide a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check) or apply for these prior to commencing in the role
- Current National Police Check prior to commencement
- Hold a current Queensland driver's license
- Provide any tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human Services)
