Lead Care Manager Opportunity

We are seeking an experienced and skilled Care Manager to join our team at My Home Care Group. As a key member of our management team, you will be responsible for delivering high-quality care services to our clients in Cairns.

Key Responsibilities:
  • Maintain the highest standards of care delivery by our home care providers and staff.
  • Lead from the front in this management position, driving excellence and innovation in care services.
  • Develop, monitor, and reassess care plans in consultation with clients, their families, and other healthcare professionals.
  • Collaborate with our nursing team, regional managers, and portfolio managers to shape our service offerings and ensure compliance with industry standards.
  • Manage and develop a team, including the Assistant Care Manager and support staff, to deliver exceptional client service.
  • Monitor care package budgets and ensure clients understand service fees.
  • Manage client service delivery across all levels, including government-funded CHSP and home care packages programs.
  • Engage with internal teams to facilitate client onboarding and lead to conversion processes.
  • Utilize influencing and partnering skills to lead and work collaboratively with schedulers, clinical teams, and support staff.
  • Ensure compliance with all relevant legislative and industry standards.
  • Proactively engage in continuous improvement across practice and service delivery.
Requirements:
  • Proven recent experience in case management in the Aged Care or Community Services sector.
  • Clinical experience preferred.
  • Knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards desired.
  • Frontline management skills.
  • Current First Aid Certificate and current CPR certificate.
  • The right to work in Australia.
  • A current driver's license, registration, and access to your own car with comprehensive car insurance.
  • National Police Clearance.
