Ozcare is a leading provider of aged care services, committed to delivering high-quality care to our clients. We are seeking an experienced Care Manager to lead client care and support across various programs, coordinating with multidisciplinary teams and services to ensure effective care.

Your Key Responsibilities:

  • Develop and implement care plans tailored to individual client needs, ensuring the highest standards of care and compliance with relevant legislation and organizational policies.
  • Liaise with clients, families, and healthcare professionals to ensure comprehensive care and foster strong relationships with clients, stakeholders, and team members.
  • Monitor and evaluate care services to continuously improve quality and efficiency, ensuring clients understand their funding and fees.
  • Manage and coordinate care services, including Home Care Packages, and ensure compliance with all relevant legislation and organizational policies.
  • Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough), and hold a current Police Screening Certificate.
  • Hold a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check), and a current Queensland driver's licence.
  • Completed tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services).

  • Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
