
Dual Diagnosis Case Manager

Role Summary
">Job DescriptionWe are seeking a dedicated individual to provide case management and counselling services to clients with dual diagnoses of substance use and mental health issues.
- The successful candidate will work closely with clients, their families, and other healthcare professionals to develop and implement individualised care plans.
- They will also utilise their skill and expertise to make assertive linkages for clients in the rehabilitation centre to ensure they receive appropriate services on discharge.
This is a 64 hours per fortnight, Monday to Friday position that requires the ability to work collaboratively as part of a multidisciplinary team. The successful candidate will have excellent communication and interpersonal skills, as well as the ability to maintain confidentiality and handle sensitive information.
