Job Title: Dual Diagnosis Rehabilitation Centre Coordinator

This role is a key position within the Westside Lodge Dual Diagnosis Rehabilitation Centre (DDRC), responsible for coordinating all intake, assessment and admission management.

The DDRC provides a supportive and safe inpatient environment for individuals aged 18 years and over requiring a client-centred, recovery oriented rehabilitation program. The programme aims to promote positive lifestyle choices and linkages to post-rehabilitation community-based programs and services are a crucial component of this initiative.

  1. Receiving assessments from service providers and collecting additional information to inform treatment planning and timely admission to WSL DDRC.
  2. Completion of screening and assessment tools in collaboration with community service providers to facilitate effective treatment plans that support clients prior, during and post-admission.

In this role, you will work closely with the DDRC Social Worker to make assertive linkages to support client admission into and discharge from WSL DDRC. Your primary focus will be on ensuring seamless transitions between pre-admission, inpatient care and post-discharge support, providing the highest level of patient care.
