Personal Care Worker

We are seeking a skilled Personal Care Worker to join our dedicated team. As a key member of the Residential Care team, you will provide professional, person-centered care and support to residents in accordance with their individual care plans, policies and procedures.

The successful candidate will work collaboratively with the clinical care team to foster maximum resident independence and autonomy. You will be responsible for providing care and daily living services to residents that helps them achieve maximum independence and meets their personal, emotional, psychological, cultural and spiritual needs.

  • Key Responsibilities:
  • Provide care and daily living services to residents that promotes their independence and meets their personal, emotional, psychological, cultural and spiritual needs.
  • Work collaboratively with the clinical care team to develop and implement individual care plans that meet the unique needs of each resident.
  • Provide support to residents in their daily living requirements, including nutrition, mobility, personal hygiene, social interaction, lifestyle activities and other support as documented within their care plan.
  • Dispense medication as directed by the Clinical Care team, as appropriately credentialed and assessed.
  • Undertake timely and accurate charting, exception reporting and other documentation as required.
  • Establish and maintain trusting and professional relationships with residents, ensuring all interactions with residents are respectful, and supportive of their privacy, dignity, individual choice and decision making.
  • Support and participate as required in a range of group services and activities, including assisting with meal service and encouraging residents' participation in activities and with feedback mechanisms.
Requirements
  • Minimum qualification of Certificate Level III in Individual Support (Aged Care) or equivalent.
  • Specific work requirements include weekend work, evening shifts and public holidays.
  • Proof of eligibility to work in Australia is required.
  • A satisfactory Police Check is required - we will support successful candidates in this process.
  • A Working with Children Check is required - we will support successful candidates in this process.
  • An NDIS Worker Screening check is required – we will support successful candidates in this process.
  • A Statutory Declaration to confirm past criminal history is required as per the Aged Care Act - we will support successful candidates in this process.
  • Aged Care Banning Orders Register checks shall be undertaken.
