Job Title: Renal Care Coordinator Allied Health

Job Summary:

We are seeking a highly skilled and dedicated Renal Care Coordinator Allied Health to join our team at Central Australian Aboriginal Congress Aboriginal Corporation. As a key member of our multidisciplinary team, you will work collaboratively with Aboriginal clients and health professionals to provide culturally safe care services for clients with complex chronic conditions of end-stage renal disease requiring haemodialysis.

Key Responsibilities:

  • Develop and implement individualized care plans to enhance the health and wellbeing of Aboriginal clients with significant chronic conditions and receiving dialysis.
  • Collaborate with healthcare professionals to facilitate management of chronic disease(s) and provide education and support to clients and their families.
  • Conduct regular assessments and reviews to ensure the effectiveness of care plans and make adjustments as needed.
  • Work closely with the Aboriginal community to promote health and wellbeing and address health disparities.

Requirements:

  • Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner with at least 2 years' post-graduate work experience.
  • Excellent understanding of primary health care and sound clinical knowledge of chronic disease management.
  • Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control.
