Diabetes Educator Role

In this dynamic and rewarding role, you will lead specialist care for individuals living with diabetes and those requiring cardiac rehabilitation in rural communities. As a Diabetes Educator CNS2, you will work collaboratively with a dedicated multidisciplinary team to deliver high-quality patient-centered care.

About the Position
  • You will provide individualized case-managed care for patients living with diabetes and those requiring cardiac rehabilitation.
  • You will be responsible for delivering community and acute support services across various locations.
  • You will participate in outreach services to support multiple communities.
  • You will work collaboratively as part of a clinical program, alongside other healthcare professionals.
  • You will contribute to the development of high-quality patient care within your specialty area.

You will receive comprehensive support, including mentoring, online learning resources, and professional development opportunities to enhance your practice and skills.

Your Qualifications and Skills
  • You are a motivated and autonomous clinician who is passionate about making a meaningful impact in rural health.
  • You hold postgraduate qualifications in Diabetes Management and/or Chronic Care (or are committed to completing these within 12 months).
  • You have at least 3 years' experience in postgraduate care (consideration may be given to those with less experience, subject to qualifications and progression pathway).
  • You possess strong communication and collaboration skills, with a commitment to delivering patient-centered care.
