Community Health Nurse

We are seeking an experienced Registered Nurse to join our Community Health team at Amplar Health. As a Community Health Nurse, you will be responsible for delivering high-quality nursing care to patients in their homes, promoting health and well-being, and providing education and support to patients and their families.
Key Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessments, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive care.
- Provide education and support to patients and their families on health-related topics, including disease management, medication adherence, and healthy lifestyle choices.
- Conduct home visits to assess patient needs and provide care, as needed.
- Document patient information and care plans in a timely and accurate manner.
- Current unrestricted AHPRA registration as a Registered Nurse.
- Working with Children Check/DCSI or equivalent.
- Current driver's license and reliable vehicle.
- 2 years' recent acute experience and ability to spot client deterioration.
- Experience in PICC care, dressing, and removal.
- Chronic wound care experience is a bonus, as well as cannulation skills.
- Competent IT skills and literacy.
- Up to 5 weeks' annual leave (for full-time and part-time employees).
