We are seeking an experienced Registered Nurse to join our Community Health team at Amplar Health. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care and improving health outcomes in the community.
Key Responsibilities- Provide comprehensive nursing care to patients in their homes, focusing on prevention, early intervention, and management of chronic conditions.
- Work autonomously, making clinical decisions and taking responsibility for patient care.
- Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
- Collaborate with multidisciplinary teams, including GPs, allied health professionals, and other healthcare providers, to ensure seamless care coordination.
- Conduct home visits, assessing patients' physical and emotional needs, and providing education and support to promote self-management and independence.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in community health nursing.
- Current unrestricted AHPRA registration as a Registered Nurse.
- Working with Children Check/DCSI or equivalent.
- Current driver's license and reliable vehicle.
- 2 years' recent acute experience and ability to identify patient deterioration.
- Experience in PICC care, dressing, and removal.
- Chronic wound care experience and cannulation skills are highly desirable.
- Competent IT skills and literacy.
- Up to 5 weeks' annual leave (for full-time and part-time employees).
