About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Hope Community Services. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality healthcare services to young people aged 12-25 years.

Key Responsibilities
  • Provide person-centered support, education, referral, and advocacy to young people regarding their physical, sexual, mental, and psychosocial health.
  • Work closely with key stakeholders and the Community Engagement team to promote health and wellbeing in the community and within secondary schools.
  • Build capacity within the multidisciplinary team by providing secondary consultation.
  • Develop and implement health education programs and interventions to optimize the health and wellbeing of young people.
  • Collaborate with other healthcare professionals to provide comprehensive and coordinated care to young people.
Requirements
  • Current Registration as Registered Nurse (Division 2) with AHPRA with tertiary qualifications in Nursing (RN Yr 2 +).
  • Registered Nurse with current AHPRA (Australian Health Practitioner Regulation Agency) registration.
  • Certificate of sexual Health /or sexual health training essential.
  • Experience working with young people plus demonstrated knowledge of and sensitivity to the mental health needs of young people (12 – 25 years) and their families and friends.
  • Minimum 2 years post graduate nursing experience working in a mental health area.
  • The ability to autonomously provide comprehensive nursing services in the primary health setting.
  • Knowledge of best practice interventions and models of care for young people.
  • Demonstrated ability to provide high quality physical and mental health assessments with young people.
  • Demonstrates ability to work effectively in a multi-disciplinary team.
  • Knowledge and understanding of relevant legislation, government policies and strategic directions in relation to mental health and alcohol and other drug disorders, particularly as it relates to young people.
  • Works to develop additional knowledge and skills.
  • Highly developed verbal and written communication skills.
  • Uses various styles of communication to ensure that information being delivered is tailored to the audience.
  • Ensures that communication is factual, accurate and presented in a clear and concise manner.
  • Conversant with relevant community based mental health terminology.
  • Demonstrates an understanding of organisational policies and processes and implements them appropriately.
  • Demonstrated understanding of the challenges and experiences of young people from diverse backgrounds including young people who are culturally and linguistically diverse and Aboriginal and Torres Strait Islanders.
  • Broad knowledge of the range of services available to young people in the local government, health, education, employment, and youth sectors.
  • Competence regarding information technology, software packages and willingness to be trained and implement the headspace data collection tool.
