We are seeking a dedicated professional to join our team as a Housing and Wellbeing Case Manager.

About the Role

This role involves providing case management services to individuals and families in crisis accommodation, street outreach, and refuges. The successful candidate will work closely with other agencies to ensure that clients receive comprehensive support and services.

Key Responsibilities
  • To assess client needs and develop individualized plans to address their housing and wellbeing needs.
  • To provide ongoing support and advocacy to clients to ensure they have access to the resources they need to achieve their goals.

Required Skills and Qualifications

We are looking for someone who has:

  • A current Cert IV in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Child and Family Intervention, or a related field.
  • At least 12 months experience in a case management role.
  • Excellent communication and interpersonal skills.
  • A strong understanding of the complex issues affecting Indigenous communities.
  • Ability to work in a culturally sensitive manner.
  • A current National Police Check and Blue Card or ability to obtain.
