Job Title: Housing and Wellbeing Specialist

  • Provide tailored supports and interventions based on the program model of care, prioritizing client agency and self-efficacy.">
  • Foster high-quality relationships with clients, grounded in empathy and respect, consistent with organizational values and vision.">
  • Maintain accurate and comprehensive case management practices, adhering to operational requirements.">
  • Employ a proactive outreach approach to engage clients, promoting seamless transitions into service delivery.">
  • Collaborate with clients to develop personalized Care Plans, addressing family goals and aspirations.">
  • Design and deliver targeted interventions, including family group meetings, to facilitate client progress.">

This full-time, permanent position offers a chance to work alongside a collaborative team of dedicated professionals. A current driver's license and Working with Children's Card (Blue Card) or eligibility to apply are required. Additionally, flexibility to work outside standard business hours may be necessary.

Requirements:

  • A Current Certificate IV in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Child and Family Intervention, or equivalent, with at least 12 months of experience in case management.">
  • Alternatively, a Current graduate university bachelor's degree in social work, human services, social science, health, allied health, youth work, community services, criminology, with at least a 10-week-full-time placement in social service field.">
  • Ability to build client agency, self-efficacy, and facilitate change through client empowerment, while actively discouraging client dependence on supports.">
  • Demonstrated knowledge and ability to provide behavior change interventions that build client self-awareness and active change in achieving their identified goals.">
  • Ability to work with the whole-of-family, irrespective of primary client, in meeting the totality of family needs.">
  • Experience with case management for high-risk and/or crisis clients, with high-level planning and organizational skills.">
  • Knowledge of the supports available to target clients in the areas of mental health, substance misuse, education, employment, and housing.">
