Job Title: Care Coordinator

Right at Home, a leading provider of in-home care services, is seeking an experienced and dedicated professional to support our growth. We are looking for a skilled Care Coordinator to join our team and contribute to delivering high-quality client care.

The ideal candidate will have a strong background in Community Aged/Disability Care and possess excellent assessment, planning, and coordination skills. The successful applicant will be responsible for service development, coordination, and delivery, including ongoing client assessments and the development of personalized care plans.

Responsibilities:
  • Service Development, Coordination, and Delivery
  • Ongoing Client Assessments
  • Care Plan Development and Implementation

This role requires a proactive and flexible individual who can work effectively in a close-knit team environment. The successful candidate will thrive on delivering exceptional client care and have a passion for making a positive impact in their local community.

Qualifications:
  • Minimum of Certificate IV Aged Care
  • Diploma in Nursing with current AHPRA registration (highly desired)
  • Current First Aid & CPR
  • Driver's License
Preferred Skills:
  • Good understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary)
  • Good understanding of the hospital discharge environment
  • Strong leadership and communication skills
