Job Description

A Care Coordinator role has become available in our Far North Queensland office. The successful candidate will have a strong background in Community Aged/Disability Care and be responsible for:

  • Developing, coordinating, and delivering high-quality care services
  • Ongoing client assessments and personalized care plan development
  • Effective case management and customer service delivery
  • Providing personal care support where required
  • General administrative tasks

The ideal candidate will thrive in a close-knit team environment, possess excellent communication and staff management skills, and demonstrate good judgment with quick decision-making abilities.
