Delivering exceptional care and support is at the forefront of this role. As a key member of our team, you will be responsible for developing and coordinating personalized care plans that cater to the unique needs of our clients.

This position requires a strong background in aged/disability care and an ability to thrive in a collaborative environment. You will work closely with our clients and their families to understand their goals and aspirations, and develop tailored care plans that promote independence and quality of life.

Some weekend supports may be required, but these will be offset during the week. Your responsibilities will include case management, customer service, personal care work, and general administration.

  • Care Assessments;
  • Government-Compliant Care Management Plans;
  • Holistic outcomes-driven Care Planning;
  • Operation of Care Planning and other management systems;
  • Personal care supports; and
  • Quality assurance.
Qualifications
  • Minimum Certificate IV Aged Care;
  • Diploma in Nursing with current registration highly desired;
  • Current First Aid & CPR; and
  • Driver's License.
Preferred Skills
  • Good understanding of Government funding environments (primarily CDC) & NDIS;
  • Good understanding of hospital discharge environments;
  • Strong teamwork, caring, and quality service culture;
  • Proven leadership abilities, professionalism, and respect for staff, clients, and families;
  • Good judgment, quick decision-making, excellent communication, staff management, and administrative skills;
