- Provide coordinated support to ensure families are connected to appropriate services.
- Collaborate with clinicians, NGOs, and Community Managed Organisations (CMOs) to establish referral pathways.
- Support families across various care settings, including hospitals, community services, and telehealth/telephone-based engagement.
- Facilitate a smoother experience for families navigating health and social service systems.
- Contribute to developing culturally safe and accessible services for all families.
- Experience working in health, mental health, or social/community services, ideally with children, young people, and families.
- Strong understanding of neurodevelopmental, behavioural, and mental health challenges in children and adolescents.
- Knowledge of local service systems and ability to collaborate across sectors.
