Central Coast Local Health District (CCLHD) is seeking a compassionate and skilled **Care Navigator** to join the **Paediatric Behavioural Triage**. Working within the Women Children and Families Directorate, this role is essential in helping children, young people, and their families access appropriate care and support pathways.
**About the Role**:
As a **Care Navigator**, you will play a key role in improving access to services for children and young people (and their parents/carers) with suspected co-morbid neurodevelopmental, behavioural, and mental health issues. You will:
- Provide coordinated ‘service navigation’ support, ensuring families are connected to the most appropriate services.
- Collaborate closely with clinicians, NGOs, and Community Managed Organisations (CMOs) to establish and maintain referral pathways.
- Support families across various care settings, including hospitals, community services, and through telehealth and telephone-based engagement.
- Facilitate a smoother experience for families navigating the health and social service systems.
- Contribute to the development of culturally safe and accessible services for all families.

- Dietitian Level 3
- Occupational Therapist Level 3
- Physiotherapist Level 3
- Social Worker Level 3
- Speech Pathologist Level 3

**About You**:
- Experience working in health, mental health, or social/community services, ideally with children, young people, and families.
- Strong understanding of neuro developmental, behavioural, and mental health challenges in children and adolescents.
- Knowledge of local service systems and the ability to collaborate across sectors (government, NGO, CMO).
