Job Summary
- We are seeking an experienced Social Worker to join our multidisciplinary team in delivering high-quality, person-centred support to in-patients and their families and carers.
About the Role
- This role involves providing rehabilitation services and discharge planning for older people in a coordinated manner;
- Working as part of a multidisciplinary team to achieve quality improvements in social work and healthcare services;
- Coordinating with various stakeholders to ensure seamless care delivery;
Your Responsibilities
- To provide high-quality, person-centred support to in-patients and their families and carers during their stay;
- To plan and coordinate appropriate services for discharge in conjunction with the multidisciplinary team;
- To contribute to quality improvements in social work and healthcare services in line with organisational objectives.
Your Qualifications and Skills
- Hold a relevant qualification in Social Work or a related field;
- Have experience working in a similar role or sector;
- Demonstrate excellent communication and interpersonal skills;
