Key Responsibilities:

  • Develop and implement personalized support plans to meet the unique needs of our customers
  • Build and maintain strong relationships with customers, their families, and other stakeholders
  • Conduct risk assessments and develop strategies to mitigate risks
  • Collaborate with our care team to ensure seamless support and services
  • Provide emotional support and guidance to customers and their families

Requirements:

  • Proven experience in support coordination or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and analytical skills
  • Current and valid Australian driver's license and well-maintained vehicle
  • NDIS Workers Screening Check and Working with Children/Working with Vulnerable People check
