About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our team at Help at Hand Support Services. As a Care Coordinator, you will play a vital role in ensuring the smooth running of our support services and providing exceptional care to our participants.

Key Responsibilities
  • Provide effective guidance and mentoring to Support Workers to ensure they have the necessary skills and knowledge to deliver high-quality care.
  • Efficiently allocate and roster Support Workers to meet the needs of our participants, ensuring that their care plans are implemented and their goals are achieved.
  • Process the onboarding and offboarding of staff and participants, ensuring a seamless transition and minimizing disruptions to our services.
  • Develop and maintain effective relationships with participants and Support Workers, fostering a positive and supportive environment.
  • Support the participant's Liaison Manager in addressing customer feedback and complaints, ensuring that our services meet the highest standards.
  • Actively participate in the recruitment strategy, ongoing recruitment process, and orientations, helping to build a strong and diverse team.
  • Promote and maintain a safe work environment, adhering to our Work Health & Safety policy and ensuring the well-being of our staff and participants.
About You

To be successful in this role, you will possess:

  • Experience and exposure to coordinating and allocating support services to individuals in their homes and community.
  • Experience with rostering using digital rostering software.
  • Exceptional customer service skills, patience, and commitment to delivering high-quality care.
  • The ability to build and maintain rapport with stakeholders, including participants, Support Workers, and other team members.
  • Possess a high level of computer literacy and experience using information management systems.
  • Tertiary qualifications in human services, social science, or a related field (advantageous).
  • Unrestricted working rights in Australia.
  • The willingness to secure a Working with Children Check if required.
Desirable Qualifications
  • University qualifications in a relevant field.
