Clinical Coordinator

We are seeking a highly skilled and dedicated professional to join our Adolescent Inpatient Unit as an Intake and Transitions Clinician. As a key member of our team, you will play a vital role in ensuring the smooth transition of young people through our inpatient unit.
Key Responsibilities- Manage incoming referrals to the adolescent inpatient unit, ensuring timely access to mental health services.
- Collaborate with the interdisciplinary team to support consumer flow through the inpatient unit.
- Work closely with external referrers and other health services to ensure seamless transitions.
- Ensure accurate and up-to-date information management systems.
- Complete post-discharge contacts within required timeframes.
To be successful in this role, you will need to possess:
- A relevant professional qualification in a health-related discipline (Nursing, Social Work, Occupational Therapy, or Psychology) and current registration with the relevant professional board.
- Strong communication and interpersonal skills, with the ability to build and maintain strong working relationships.
- Excellent organizational and time management skills, with a dynamic and flexible approach to caseload demands.
- A flexible, positive, and innovative team-oriented approach to service delivery and change.
