We are seeking a highly skilled and dedicated professional to join our Adolescent Inpatient Unit as an Intake and Transitions Clinician. As a key member of our team, you will play a vital role in ensuring the smooth transition of patients through our inpatient unit.
Key Responsibilities- Manage incoming referrals to the adolescent inpatient unit, including recording relevant information and liaising with the inpatient team regarding admissions.
- Work collaboratively with the interdisciplinary team, external referrers, and other health services to ensure timely access to adolescent mental health inpatient beds.
- Support consumer flow through the inpatient unit, ensuring safe and successful discharge for patients back into the community setting.
- Ensure relevant information management systems are kept up to date.
- Complete post-discharge contacts within required timeframes.
To be successful in this role, you will need to bring a strong background in a health-related discipline, such as nursing, social work, occupational therapy, or psychology, and current registration with the relevant professional board. You will also need to demonstrate a flexible, positive, and innovative approach to service delivery and change, as well as strong communication and interpersonal skills.
