Austin Health is seeking a skilled and compassionate Nurse Specialist - Community Care to join our team. As a key member of our Transition Care Program, you will play a vital role in delivering high-quality, patient-centered care to individuals in our community.
Key Responsibilities:
- Manage the clinical needs of patients in the Transition Care and Restorative Care programs, ensuring effective communication and clinical competency to support patients' journeys.
- Work closely with key stakeholders at our Residential Care facility and brokered service providers to support effective care and discharge planning.
- Escalate clinical deterioration and implement risk mitigation strategies to ensure safe and effective care.
- Develop close working relationships with referrers, specialist clinics, and services to ensure seamless care coordination.
Requirements:
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia.
- Strong clinical skills and demonstrated experience in an aged care or community setting.
- Excellent interpersonal and communication skills that support patient care, team engagement, and collaboration.
- A willingness to think creatively and outside the square to achieve the best possible client outcomes.
- Demonstrated flexibility to change and adapt with program and organizational requirements.
