Care Coordinator

Coordinating client care is a critical function that requires an exceptional individual to oversee all allocated client activities. This role entails ensuring high-quality consumer-directed care delivery, addressing daily inquiries and requests, providing education on available services, and monitoring the quality of care.

Key Responsibilities:

  • Recruit, train, and supervise carers to maintain service standards.
  • Manage operational performance through scheduling and monitoring.
  • Develop relationships with clients and their families by handling inside sales enquiries.

Responsibilities Continue:

  • Process daily client inquiries and requests in a timely manner.
  • Provide educational support on available services and care plans to clients and their families.
  • Monitor the quality of care delivered to clients.
  • Collaborate with colleagues to provide holistic client-centered care.
  • Maintain accurate records and documentation of client interactions.

Requirements:

  • Hold a Certificate III in Aged Care or equivalent qualification.
  • Desirable: relevant tertiary qualification or prior experience in aged care.

Successful candidates will possess excellent communication and interpersonal skills, work effectively in teams, and be committed to delivering high-quality care. They will work primarily in clients' residences, requiring flexibility with varying shift times and days.
