About the Role:
The Case Manager will coordinate a team that works with children and families to provide high-quality clinical care and case management using a holistic approach.
A regular day will see you:
- Supporting referrals and admissions for children and their families to the Child Inpatient Unit in a multi-disciplinary setting, with extensive preadmission work up, comprehensive biopsychosocial assessment and treatment case on admission.
- Providing high quality secondary consultation to families, and to primary and other specialist service providers, on behalf of the team.
- Providing accurate, accessible and timely written records of clinical work, as required in CYMHS standards of documentation.
Requirements:
You will be someone who has excellent communication and interpersonal skills, with the ability to develop and maintain effective working relationships with consumers, families/carers, colleagues, clinical staff, and other health and welfare services.
To be successful in this role you'll bring with you:
- A clear understanding of and practice in the principles of recovery-orientated trauma informed and patient focused/family-centred care,
- Collaborative practice and expertise in engaging children and their families within a clinical setting.
- Ability to work collaboratively within a multidisciplinary team, including expressing own viewpoints respectfully and valuing the perspectives of others.
- Awareness and practice knowledge of relevant legislation and practice frameworks (e.g. Mental Health Act and Child Youth and Families Act).
