Job Title: Clinical Care Specialist
Overview:
The Clinical Care Specialist will be responsible for conducting comprehensive assessments and providing treatment planning, referrals, and linkages into community services. This role is the first point of contact for clients seeking to access alcohol and other drug services.
Key Responsibilities:
- Receiving and managing stakeholder referrals for all programs
- Undertaking comprehensive assessments and developing clinically appropriate treatment plans
- Waitlist management for residential inpatient units
- Contributing to skilled case coordination services as part of a multidisciplinary team
Requirements:
Qualifications in social work, nursing, psychology, or other relevant health discipline. Excellent assessment, treatment planning, and clinical presentation skills. Ability to manage competing priorities within a multidisciplinary team.
