
Personalised Disability Support Specialist

This role involves providing personalised support to individuals with disabilities, focusing on those with mental health needs.
The ideal candidate will work in a client's home and within the community, assisting with daily living activities, access to community resources, and transportation.
Key qualifications include a strong background in community participation and personal care, ability to build meaningful relationships, and make a positive impact on clients' lives.
Preferred candidates possess excellent communication and interpersonal skills, a willingness to learn and develop new skills, and the ability to work independently as well as part of a collaborative team.
We are seeking a candidate from a Maori or Pacific Islander background who is enthusiastic, easy-going, and has experience working with individuals with diverse needs.
Responsibilities:
- Providing one-on-one support and assistance to clients with varying needs
- Working within a person-centred environment that respects clients' rights and needs
- Assisting with daily living activities, including personal care, meal preparation, household tasks, and domestic duties
- Accompanying clients on daily errands, offering companionship, and support
- Supporting clients with academic, social, or recreational activities
- Preparing meals, feeding clients, and assisting with medication administration
- Monitoring clients' physical and emotional well-being, reporting any changes
- Engaging clients in social and recreational activities to foster independence and community integration
- Maintaining accurate records and documentation as required
- Collaborating with a multidisciplinary team to ensure effective delivery of services
