The Way Back Support ServiceThe Way Back Support Service is a non-clinical peer support program offering up to 3 months of support for people who have recently attempted suicide or experienced a suicidal crisis. It's built on personal connection and integration with community services.
Key Responsibilities- We provide direct and personalised assistance to consumers who have survived a suicide attempt or are experiencing a suicidal crisis by providing referrals and linking with appropriate services, monitoring and reporting progress.
- We use personal lived experience of mental health recovery to connect with consumers and provide support in a recovery-oriented, person-centred, and strengths-based way.
- We provision and documentation of mental health assessments, including completion of support planning, safety planning and mandatory reporting as required.
- We consult, liaison and linkage with internal and external service systems to provide a full range of tailored supports, including collaboration with Hospital and LHD Mental Health teams.
- We provide regular, flexible, peer support to consumers, and assist them to review their needs and resources and monitor activities identified in Individual Support Plans and Safety Plans.
- A minimum of a Certificate IV in a Community Services related discipline (ideally Peer Work)
- We hold, or be willing to work towards, a Certificate IV in Peer Work and complete Safe Story Telling training or similar as identified
- Personal lived experience of suicide or suicidal crisis recovery and proactivity in managing wellbeing; or lived experience of supporting a loved one
- A minimum of 2 years' experience working with people at risk of suicide, and supporting people with mental health challenges
