Job Title: Housing and Wellbeing Advocate

Job Description:

The role of the Housing and Wellbeing Advocate is to empower individuals and families to overcome their housing and wellbeing challenges. This will be achieved through partnership working, outreach engagement, and the provision of high-quality case management practices.

Key responsibilities of this role include:

  • Providing targeted interventions and support to meet individual client needs.
  • Developing and implementing client-led care plans that promote positive change and empowerment.
  • Working collaboratively with clients to identify and address barriers to housing and wellbeing.
  • Facilitating access to holistic care through coordination of multiple services and agencies.

Additional responsibilities include:

  • Providing regular reporting and updates to stakeholders.
  • Participating in training and professional development opportunities to enhance skills and knowledge.
  • Engaging in ongoing evaluation and quality improvement activities to ensure best practice.
Required Skills and Qualifications:

This position requires a strong understanding of housing and wellbeing issues, as well as excellent communication and interpersonal skills. Key qualifications for this role include:

  • A relevant tertiary qualification in social work, human services, or a related field.
  • At least 12 months experience in a case management or related role.
  • Current Cert IV in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Child and Family Intervention.

Preferred skills and qualifications include:

  • Graduate university degree in social work, human services, or a related field.
  • Experience working with Aboriginal and Torres Strait Islander communities.
  • Knowledge of trauma-informed practice and trauma-sensitive service delivery.
