Job Overview

Empower individuals and families to achieve their goals by providing tailored support and interventions. The ideal candidate will work collaboratively with clients to understand their needs, identify solutions, and develop a personalized plan to overcome housing and wellbeing challenges.

Key Responsibilities
  • Deliver high-quality case management practices and record keeping that meet operational requirements.
  • Facilitate client-led assessments and care plans to identify goals and develop targeted interventions.
  • Provide role-specific supports and family group meetings to achieve client outcomes.
Requirements
  • Current qualification in social work or a related field, such as a Cert IV in Social Science, Health, or Community Services.
  • Experience working with high-risk and/or crisis clients, with strong planning and organisational skills.
  • Knowledge of available supports for mental health, substance misuse, education, employment, and housing.
Desirable Skills and Experience
  • Immunisation history showing two doses of Covid-19 vaccine.
  • Working with Children's Card (Blue Card) or eligibility to apply.
  • Current National Police Check.
