Responsibilities
  1. Support participants with complex needs to engage in activities and tasks including capacity building, in-home support, and community access.
  2. Provide direct assistance to each participant in accordance with their support/service plan and as required.
  3. Work in accordance with medication administration policy, work health and safety procedures, and client rights policies.
  4. Monitor the safety and well-being of the participant and report any concerns to the Care Coordinator or Practice Lead.
  5. Ensure completion of appropriate documentation using the Client Management System.
