We are seeking a skilled and compassionate individual to join our team as a Holistic Community Support Specialist. In this dynamic role, you will work collaboratively with participants and their families to provide outreach wellbeing support and empower them to thrive in their local communities.

Key Responsibilities:

  • Provide case management, assertive outreach, and group facilitation to support participants and their families in their own home and within their community
  • Develop and maintain professional relationships with participants and all relevant stakeholders
  • Provide short-term assistance to enable participants to work towards their goals
  • Increase self-management and independence for participants

Requirements:

To be successful in this role, you will need:

  • Prior lived or living experience supporting people with dual diagnosis or dual disability
  • Able to work autonomously to identify individual and family needs
  • Effective listening skills and an adaptable communication style
  • Capable of advocating for others and building appropriate support networks
  • Able to contribute positively within a team, sharing ideas and working collaboratively
  • Able to source suitable and sustainable supports and services for participants

The estimated salary range for this role is $39.26 - $42.25 per hour, depending on experience and qualifications. This position also offers salary packaging options and flexible employment conditions.
