Disability Team Leader Role

We are seeking an experienced Disability Team Leader to oversee operations within assigned houses and manage team members to ensure quality care for clients.

The successful candidate will coach and mentor support workers on best practices in disability care.

Key Responsibilities:
  • Compliance and Documentation: Ensure team compliance with documentation, including Progress Notes, Incident Reports, Medication Signing Sheets & Charting.
  • Medication Management: Update and administer client medications based on established standards.
  • Care Plan Compliance: Maintain team compliance with care plans, organizational policies, and procedures.
  • Client Concerns: Conduct investigations into client concerns and facilitate valid complaints.
  • Team Attendance and Punctuality: Ensure team attendance and punctuality.
  • Housing Checks: Conduct regular house checks.
  • Communication: Serve as primary communication channel between staff, management, and clients.
  • Client Onboarding: Facilitate client onboarding, booking, and appointment setting.
Requirements:
  • Experience: Proven work experience in community or disability sector.
  • Driver's License: Valid Australian driver's license.
  • Language Proficiency: Proficient in English language.
  • Clearance and Certifications: National Disability Insurance Scheme (NDIS) Clearance Check, First Aid & CPR certificate, Medication Competent, Manual Handling training.
  • Flexibility: Flexible and willing to take on various challenges.
  • Availability: Able to report to work onsite at the physical office (Maylands) and assigned houses.
  • Qualifications: Valid Certificate III or Diploma related to Individual Support, Disability, or equivalent.
  • Schedule: Available to work 5 days a week - morning shift.
  • Computer Literacy: Computer literate.
  • Software Skills: Knowledgeable in Microsoft Office and Google Workspace.
