Job Overview

The Care Coordinator role is integral to the organization's mission of providing exceptional patient care. This position involves participating in ongoing quality improvement initiatives, conducting thorough assessments of patient progress post-discharge, and generating reports on key performance indicators.

Key Responsibilities:
  1. Quality Improvement: Participate in continuous quality improvement activities to enhance patient outcomes.
  2. Post-Discharge Follow-up: Conduct regular follow-ups to assess patient progress, address concerns, and prevent readmissions.
  3. Reporting: Generate reports on key performance indicators, client outcomes, and other relevant metrics to inform decision-making.
  4. Data Analysis: Analyze data to identify trends, areas for improvement, and opportunities for optimization.
  5. Referral Management: Manage the referral process, including receiving and processing referrals from healthcare professionals or agencies.
  6. Communication: Ensure timely communication and coordination with referring entities.
  7. Client Records: Maintain accurate and up-to-date client records, including care plans, assessments, and communication logs.
  8. Compliance: Ensure compliance with privacy regulations and organizational policies.
  9. Collaboration: Collaborate with physicians, nurses, therapists, and other healthcare professionals to optimize patient outcomes.
  10. Team Meetings: Participate in interdisciplinary team meetings to discuss and plan patient care.
  11. Client Communication: Maintain regular communication with clients and their families to assess satisfaction and address concerns.
  12. Care Provider Communication: Communicate with care providers to ensure they are well-informed about the clients' needs and preferences.
Requirements:

A bachelor's degree in Nursing is preferred. Previous experience in care coordination or a related role in the healthcare industry is required. Proficiency in computer applications, including Microsoft Office and care management software, is essential. CPR and First Aid certification are mandatory.
